For young people transitioning from pediatric care to adult care, there can be challenges in taking charge and advocating for their own health. Discussion in this session includea the proper time for transition of care, the transfer of medical records to a new care provider, the need of care providers to communicate effectively during the process, and the support that is necessary to address existing health needs, particularly those of a chronic disease or disability nature.
