Posterior urethral valves (PUV) are the most common cause of bladder outlet obstruction in baby boys. Our discussion deals with the natural history of the anomaly and its clinical implications for affected boys. Major points of focus include the role of fetal intervention for valves, the postnatal treatment of obstruction during the newborn period, and the subsequent treatment of bladder dysfunction and hydronephrosis that often persist through later development. In addition, a subset of boys, whose valves go undetected while in utero, present at a later age. Differentiating these from other causes of bladder outlet obstruction, both anatomic and functional, sometimes poses a diagnostic challenge that is addressed.
