This presentation addresses transitional care models, including the background and philosophy, evidence-based approaches, and outcomes associated with this unique approach to care. Specific case examples are used to highlight transitional care of patients experiencing urologic and pelvic floor disorders. Implications associated with adapting this approach to care in clinical settings and future directions are examined.
